Abstract

In the UK, general practice has historically acted as a gatekeeper service to the more specialised services offered in hospital. This divide between generalism and specialism is not immutable. Doctors, nurses and managers in general practice can learn more specialised skills. Indeed, all generalists should become more skilled as their careers develop and healthcare expectations change: some clinical generalists will develop sufficient skills to deliver services that have traditionally only been provided in the hospital setting. For clinical expertise to be developed, particular skills are needed, which can only be provided if part of the learning is through case-based teaching.
