A retractor left in the pelvic area. A laparotomy sponge that causes an abscess. Although relatively uncommon, such errors can result in serious patient outcomes, including pain, infection, and death. This month's Focus on Five identifies five risk factors that can increase the chance of surgical tools being retained in patients.
