Abstract
A patient's health care experience may not always move in a linear fashion. In some cases, patients discharged from the hospital to home care may be readmitted to the hospital due to falls, adverse medication errors, or surgical site infections. To address high rates of home care patients readmitted to hospitals, The Joint Commission has designed a tracer to assess how well a home care organization evaluates its hospital readmission risks and its steps to reduce or prevent readmission. This article reviews how you can conduct your own tracer to determine how well your process works and if it is reducing the number of preventable readmissions.
